A colleague of mine recently found that North Carolina Schools are succesfully implementing ideas that foster follow-up. They developed a self-assessment form for individuals to reflect on their own transfer of new knowledge and skills from the workshop to the classroom.

Perhaps Phase 1 of the form should be completed at the time the participants are engaged in the professional learning activity or experience. This section mirrors the Continuous Improvement Plan (CIP) used in many of the Teacher Development events.
Action Resulting= Desired State, Expected Outcome= Assessment, Resource= Support/Intervention, and Evaluation= Impact.
